Each of the following abatements have been reviewed by a Utah State University authorized representative to make sure they are accurate, and that all affected employees and their representatives have been informed of the abatement activities described in each document. 

By signing the document, the employer does not admit that it violated the cited standards or any litigation or purpose other than subsequent proceeding under the UOSH Act. 

Inspection 1419367, Citation 2 Item 1

Utah State University now requires Hepatitis B Vaccination Informed Consent/Waiver Form for all personnel that are deemed “reasonably anticipated to have a skin, eye or mucous member contact with blood or bodily fluids” in the performance of their job. These forms are provided to all attendees of initial and refresher Blood Borne Pathogens training. USU departments will maintain these forms and give personnel an opportunity to modify their declaration at any time and during refresher training. USU Departments will provide vaccinations to anyone that desires to be vaccinated and has not been. The Informed Consent/Waiver Form is being sent to all campus units that may have already received training without getting the Form.

  • USU Facilities LOTO Procedures

Procedure was developed to ensure blade change or maintenance service to machine – To be locked out and tagged out.

  • 8 hour time-weighted average exposure to chromium (VI)
    • Inside energy plant
    • Welding on stainless steel

USU Facilities has abated the violation by performing TWA personal air sampling on Ben Clark and Justin Jones at the Central Energy Plant. Justus Swenson from WCF Insurance performed the air monitoring. Results from this survey determined both employees were under the action limit. Please refer to the attached survey and supporting documents. Affected employees were informed of abatement.

Inspection 1417067

Employer did not assess the NDFS lab preparing bacteria and communicate necessary PPE

In response to the Inspection Number 1417067 – Citation 1 Item 1. A faculty member, in association with the College of Agriculture and Applied Sciences and the Environmental Health & Safety Office, has abated the violation by performing an assessment of the laboratory procedures required to prepared agar plates for food bacteria analysis. The assessment determined the hazards and identified the necessary administrative, engineering and personal protective equipment necessary to reduce the acknowledged risks. The resulting Standard Operating Procedure was put into place and communicated to all persons working in the area.
